Clioquinol

Clioquinol is a powerful anti-infective drug available for topical (Vioform) and internal (Enterovioform) use. It is neurotoxic in large doses and is associated with the SMON (Sub-Acute-Myelo-Optical-Neuropath) epidemic in Japan between 1957 and 1970 that affected over 10,000 people.

Recent research at UCSF indicates Clioquinol appears to block the genetic action of Huntington's disease in mice and in cell culture.

Drug challenge

You routinely administer combination drugs...but do you know what's in them? To find out, match each combination of ingredients in Section II with its name in Section I.

SECTION I

1. Combivent (Boehringer Ingelheim)

2. Teczem (Hoechst Marion Roussel)

3. Sildec-DM (Silarx)

4. Hysone (Roberts Med)

5. Repan (Everett)

SECTION II

a. each ml contains: pseudoephedrine HCI, 25 mg; carbinoxamine, 2 mg; dextromethorphan, 4 mg These pediatric oral drops may be prescribed for a toddler's cold symptoms. Pseudoephedrine HCl is a decongestant; carbinoxamine, an antihistamine. Dextromethorphan suppresses cough. Teach the child's parents how to administer the drops.

b. each actuation delivers: ipratropium, 18 mcg; albuterol sulfate, 103 mcg

Use this aerosol oral inhaler to relieve bronchospasm in patients with chronic obstructive pulmonary disease. An anticholinergic, ipratropium inhibits vagal reflexes; albuterol, a sympathomimetic, relaxes bronchial muscles. Teach your patient to use a metered-dose inhaler.

c. each gram contains: clioquinol, 30 mg; hydrocortisone, 10 mg

A patient may select this cream to treat a fungal skin infection. Clioquinol is a topical antifungal and antibacterial agent. Hydrocortisone, a corticosteroid, relieves inflammation and itching. Tell your patient to contact his physician if the skin rash hasn't healed within 1 week.

d. acetaminophen, 325 mg; butalbital, 50 mg; caffeine, 40 mg

This capsule may be prescribed to relieve mild to moderate tension headaches. Acetaminophen is a nonopioid analgesic; butalbital, a barbiturate, is a sedative. Caffeine may enhance the analgesic effects of acetaminophen. Warn your patient that he may experience dizziness.

e. enalapril, 5 mg; diltiazem maleate, 180 mg This extended-release tablet may be prescribed, to treat hypertension. Enalapril is an angiotensin-converting enzyme inhibitor; diltiazem maleate, a calcium channel blocker. Tell your patient to contact his physician if he experiences shortness of breath or swelling of the hands or feet.
