Look at the letters
From Nursing, 10/1/98 by Cohen, Michael R

The FDA and United States Pharmacopeia recently received reports noting confusion between Soriatane (acitretin) capsules and Loxitane (loxapine succinate) capsules. Soriatane is a retinoid used to treat severe psoriasis; Loxitane is indicated to manage psychotic disorders.

The drug names may appear similar when written with cursive letters, as shown in the sample. Compounding the problem is the fact that both products are available in 10 mg and 25 mg capsules.

These medications are for very different conditions. If your patient's medication isn't appropriate for his diagnosis, call the physician to question the order.
